สมาคมฟ้าสีรุ้งแห่งประเทศไทยก่อตั้งขึ้นในปี พ.ศ. 2542 โดยการริเริ่มของคุณกมลเศรษฐ์ เก่งการเรือ นักชำนาญการสังคมสงเคราะห์ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ซึ่งต่อมาได้เป็นหนึ่งในผู้ก่อตั้ง สมาคมฯ และปัจจุบันดำรงตำแหน่งเป็นเลขาธิการสมาคมฯ ได้เล็งเห็นว่าอัตราการติดเชื้อเอชไอวีในกลุ่มชายรักชายสูงขึ้นอย่างต่อเนื่อง รวมทั้งปัญหาที่กลุ่มชายรักชายรวมทั้งผู้ที่มีความหลากหลายทางเพศ ยังถูกสังคมตีตรา เลือกปฏิบัติ ถูกผลิตซ้ำในภาพลักษณ์ที่ด้อยค่าในสังคม และด้วยเหตุผลที่ต้องการให้เกิดความเข้มแข็งในกลุ่มผู้ที่มีความหลากหลายทางเพศ คุณกมลเศรษฐ์ เก่งการเรือ จึงได้ชักชวนกลุ่มชายรักชายที่มีจิตอาสามาทำกิจกรรมที่เป็นประโยชน์ ภายใต้ชื่อ “ชมรมเส้นทางสีรุ้ง” โดยการจัดกิจกรรมแจกถุงยางอนามัยและให้ข้อมูลเรื่องการป้องกันการติดเชื้อเอชไอวีแก่กลุ่มเป้าหมายชายรักชายที่มาใช้พื้นที่สวนลุมพินีเพื่อการพักผ่อน โดยในช่วงแรกการทำงานเป็นลักษณะจิตอาสา ไม่มีงบประมาณสนับสนุนการทำงานใดๆทั้งสิ้น จนในระยะเวลาไม่นานมีผู้ให้ความสนใจมาเข้าร่วมกลุ่มและร่วมเป็นอาสาสมัครในการจัดกิจกรรมต่างๆเพิ่มมากขึ้น ในปี พ.ศ. 2543 จึงเปลี่ยนชื่อเป็น “กลุ่มฟ้าสีรุ้ง” โดยมีแนวคิดมาจากการที่สีรุ้งเป็นสัญลักษณ์ของความหลากหลายทางเพศอันเป็นสากล ซึ่งเปรียบเสมือนความหลากหลายของมนุษย์ เมื่อมารวมกับคำว่าฟ้า จึงเปิดเป็นคำว่า “ฟ้าสีรุ้ง” ซึ่งสื่อความหมายถึง ความหลากหลายที่อยู่ภายใต้สังคมเดียวกันอย่างสวยงามดั่งสายรุ้งบนฟากฟ้าที่สดใสโดยองค์กรหมอไร้พรมแดน ประเทศเบลเยี่ยม เป็นองค์กรแรกที่ สนับสนุนถุงยางอนามัยและเอกสารให้ความรู้เพื่อ ให้อาสาสมัครสามารถให้ข้อมูลความรู้กับเพื่อนๆ ในชุมชนได้ดียิ่งขึ้น จึงได้มีการเปลี่ยนสถานะจากกลุ่มฟ้าสีรุ้ง เป็น “องค์กรฟ้าสีรุ้ง” เมื่อการดำเนินกิจกรรมได้รับผลสำเร็จเป็นอย่างดี ต่อมาจึงได้รับงบประมาณสนับสนุนจากกรุงเทพมหานคร กระทรวงสาธารณสุข และสภากาชาดไทย
ในปี พ.ศ.2546 ศูนย์ความร่วมมือ ไทย-สหรัฐด้านสาธารณสุข ศูนย์วิจัยโรคเอดส์ สภากาชาดไทย และสมาคมฟ้าสีรุ้งแห่งประเทศไทย ได้ร่วมกันจัดการศึกษาความชุกของเชื้อเอชไอวี ในกลุ่มชายมีเพศสัมพันธ์กับชาย โดยพบว่ามี อัตราการติดเชื้อเอชไอวีในกลุ่มชายมีเพศสัมพันธ์กับชายในกรุงเทพฯ อยู่ที่ร้อยละ 17.3 ซึ่งนำไปสู่การยกระดับขยาย การทำงานป้องกันเอดส์ใน กลุ่มชายมีเพศสัมพันธ์กับชายในกรุงเทพมหานคร
เดือนพฤศจิกายน พ.ศ. 2546 องค์กรฟ้าสีรุ้งได้จดทะเบียนเพื่อจัดตั้งเป็นสมาคมกับกระทรวงวัฒนธรรมภายใต้ชื่ออย่างเป็นทางการว่า “สมาคมฟ้าสีรุ้งแห่งประเทศไทย” และได้ดำเนินการจัดตั้งสำนักงานภายใต้งบประมาณสนับสนุน จากศูนย์ความร่วมมือไทย-สหรัฐด้าน สาธารณสุข และการสนับสนุนงบประมาณจากองค์กร FHI 360 , PACT , กองทุนโลก , PSI , UNAIDS , UN WOMEN และ AJWS ปัจจุบันสมาคมฟ้าสีรุ้งแห่งประเทศไทยมีการดำเนินโครงการด้านสุขภาพเฉพาะสำหรับ กลุ่มชายมีเพศสัมพันธ์กับชาย คนข้ามเพศ (สาวประเภทสอง) หญิงรักหญิง รวมถึงการทำงานประเด็นเรื่องสิทธิของผู้ที่มีความหลากหลายทางเพศ และได้มีการจัดตั้งศูนย์สุขภาพฟ้าสีรุ้ง รามคำแหงและหาดใหญ่ เพื่อให้บริการตรวจเอชไอวีและโรคติดต่อทางเพศสัมพันธ์ แก่กลุ่มชายที่มีเพศสัมพันธ์กับชายแบบรู้ผลในวันเดียว พร้อมบริการดูแลสนับสนุนสู่การรักษาอย่างต่อเนื่อง ภายใต้การสนับสนุนจากองค์การเพื่อการพัฒนาระหว่างประเทศแห่งสหรัฐอเมริกา องค์กร PSI ศูนย์วิจัยโรคเอดส์ สภากาชาดไทย และการสนับสนุน ทางด้านวิชาการจากคลินิกชุมชน สีลม
ปัจจุบัน สมาคมฟ้าสีรุ้งแห่งประเทศไทย มีสำนักงานและศูนย์กิจกรรม 7 แห่งทั่วประเทศ ได้แก่ กรุงเทพฯ สมุทรปราการ ปทุมธานี นนทบุรี ชลบุรี อุบลราชธานี และสงขลา มีเจ้าหน้าที่ปฏิบัติงานมากกว่า 80 คน และอาสาสมัครมากกว่า 1,000 คน
